Bone Scintigraphy

Clinical summary Prostate adenocarcinoma, Gleason 4+4, PSA 28 ng/mL, on ADT; new lower back pain.
Clinical indication Staging / assessment for osseous metastases.
Imaging findings Multiple foci of increased radiotracer uptake in the axial skeleton (thoracolumbar spine, pelvis, ribs), asymmetric and variable in intensity. Normal renal excretion and soft-tissue clearance; no superscan pattern.
Imaging diagnosis Findings consistent with widespread osseous metastases (polyostotic). Correlate with PSA trend and, if needed, targeted CT/MRI or PSMA PET.

MRI

TNM assessment cT2cN0M0 (08-05-26)
PI-RADS 2
Laterality Right
Side Dorsal
PSA density 3.0 ng/ml
Location Peripheral zone
Segment Middle
Imaging findings The dimensions of the prostate are 4.6 x 4.0 x 4.9 cm. The area of diffusion restriction is midpolar on the right dorsal side in the peripheral zone. The diameter is 1.2 cm. Minimal capsular breakthrough. Close relationship with the left neurovascular bundle. In the transition zone, there is also an area of diffusion restriction ventrally in the midline. This picture is consistent with a BPH nodule. No lymphadenopathy. No abnormalities in the seminal vesicles. No abnormalities in the urinary bladder. No abnormalities in the skeleton.
